Penile blood flow and the mechanics of erection

How does blood flow into the penis? The corpus cavernosum, nitric oxide, the inflow–outflow balance and the causes of poor penile blood flow — plus how vacuum therapy affects circulation.

“Blood flow to the penis” is how most people describe the whole physiology of erection — and it is actually a very apt description: an erection is less a chemical event than a matter of blood volume and pressure. When enough blood flows in, the penis becomes firm; when inflow falls or outflow speeds up, an erection either does not develop or cannot be maintained.

An erection is a blood volume event

In the flaccid state, blood flow into the penis is low and the blood that arrives drains back through the veins at the same rate. Arousal changes this balance: the arteries widen, flow increases several-fold, the tissue fills, and the expanding tissue compresses the veins against the outer sheath, restricting outflow. The secret of firmness is that both mechanisms work at the same time. Increased inflow alone is not enough; if outflow is not restricted, the tissue cannot stay filled.

Inside the penis

  • Corpora cavernosa (2): the sponge-like erectile tissue that runs side by side along the upper part of the penis. This is the structure that provides rigidity.
  • Corpus spongiosum: the lower body surrounding the urethra, which becomes less rigid.
  • Tunica albuginea: the tight, minimally stretchable sheath enclosing the corpora cavernosa. As the tissue fills, the veins are compressed against this sheath, keeping the blood inside.
  • Helicine arteries: small, coiled arteries that carry blood into the spongy tissue. This is where arterial stiffening strikes first.
  • Endothelium: the layer of cells lining the inside of blood vessels. It produces nitric oxide, which starts the widening of the vessels.

How an erection happens, step by step

  1. A stimulus (visual, tactile or mental) travels from the brain through the spinal cord to the pelvic nerves.
  2. Nerve endings and the endothelium release nitric oxide (NO).
  3. NO raises cGMP levels in smooth muscle cells; the smooth muscle in the vessel walls relaxes.
  4. The helicine arteries widen, blood flow rises rapidly and the spongy tissue fills.
  5. The filled tissue presses against the tunica albuginea, the veins are compressed and outflow falls: an erection forms.
  6. After ejaculation or when stimulation stops, NO decreases, the smooth muscle contracts, outflow is released and the penis becomes flaccid.

Why does blood flow fail?

MechanismTypical causes
Reduced inflow (arterial insufficiency)Atherosclerosis, high blood pressure, high cholesterol, smoking, diabetes, advancing age.
Failure to close off outflow (venous leak)Changes in the tunica albuginea and smooth muscle structure, tissue fibrosis, Peyronie’s disease.
Signal not reaching the penis (neurogenic)Radical prostatectomy and pelvic surgery, diabetic neuropathy, multiple sclerosis, spinal cord injury.
Endothelial dysfunctionSmoking, metabolic syndrome, chronic inflammation, physical inactivity, sleep apnea.
Hormonal / central causesLow testosterone, thyroid disorders, depression, certain medications.

What happens to the tissue when blood flow drops?

This is the least discussed and most important part of the topic. Erectile tissue needs to fill with blood regularly; this is partly the purpose of the spontaneous erections that recur throughout the night. If there is not enough blood flow for a long time, tissue oxygenation falls and some of the smooth muscle cells are replaced by inelastic connective tissue. This process deepens the erection problem further; in other words, loss of blood flow gradually creates a self-reinforcing vicious circle.

The same process underlies the loss of penile length and girth reported after radical prostatectomy. If the tissue is deprived of blood during the months it takes the nerves to heal, there is less working tissue left by the time recovery arrives. The whole logic of penile rehabilitation after prostate surgery is to bridge this gap.

How does vacuum therapy affect blood flow?

A vacuum device lowers the air pressure inside the cylinder, creating a pressure difference between the tissue and the cylinder. Blood moves towards the low-pressure area and fills the spongy tissue — even when nerve signalling is weak. Once the session ends, the blood drains back; when a constriction ring is used, the erection is maintained for the duration of intercourse.

In rehabilitation use, no ring is applied at all. The goal is not an erection but regular filling and oxygenation of the tissue. This regular “fill and release” rhythm can be thought of as an external contribution to the natural role of nighttime erections. This mechanism explains how the device helps preserve penile length and girth; it is not a tool for permanent enlargement.

Habits that support healthy blood flow

  • Quitting smoking — the most noticeable and fastest gain for endothelial function.
  • Regular aerobic exercise — even 150 minutes of walking per week supports small-vessel health.
  • Keeping blood sugar, blood pressure and cholesterol on target.
  • Reducing waist circumference — lowers metabolic load and hormonal imbalance.
  • Treating sleep apnea — repeated drops in oxygen during the night wear down the vessel lining.
  • Not giving up sexual activity entirely — regular blood flow benefits the tissue; when needed, mechanical support helps preserve this continuity.

Which symptoms call for a doctor’s visit?

  • A marked decrease in, or loss of, morning erections.
  • Erectile problems that start suddenly.
  • Pain, curvature or a hardened area/plaque you can feel in the penis.
  • A painful erection lasting longer than 4 hours (priapism) — this is a medical emergency.
  • Erectile problems accompanied by shortness of breath, chest pain or leg pain when walking.
